Annual Provider Education

To meet compliance with state, federal and contractual obligations, Providers who participate in Medicaid/Medicare Advantage programs are required to complete Compliance, Fraud, Waste and Abuse, Cultural Competency, Disability Awareness, and CMS Model of care training with SB 923 Gender Affirming Care Content.

The trainings are offered below in PDF format and at the end of each training, an attestation is provided to receive credit for completing. An option to email yourself a certificate of completion is also available at the end of the Attestation.

If you have completed appropriate education and training to identify, correct and prevent potential fraud, waste and abuse, as required by 42 CFR § 442.503, 42 CFR § 442.504, Chapter 21 Medicare Managed Care Manual section 50.3, and California Health and Safety Code 1348(a) elsewhere and do not wish to complete Providence’s education and training, please review and complete this Attestation Form
