We provide education and support for preventing health problems whenever possible, and we offer expert diagnosis and treatment when needed. Our comprehensive care is delivered through a team-based approach, guided by your dedicated primary care provider.
Your primary care provider manages all your health care needs, including routine and urgent care, preventive care and referrals to specialists. Your care team may also include: a nurse practitioner, a physician assistant, a clinical pharmacist, a pharmacy technician and a clinical care coordinator.

Please arrive 15 minutes before your scheduled appointment so that you have time to check in and fill out any necessary paperwork.
Please bring the following to your first visit:
- Photo ID and insurance card
- Co-payment if required by your insurance plan
- Full list of medications with dosages, including over-the-counter supplements
- Detailed history of symptoms and treatments
- List of questions for the care team
We accept many health insurance plans and bill most insurance plans directly. Since coverage can vary, it’s best to confirm with your insurance provider directly before you schedule an appointment, so you aren’t surprised by unexpected charges.
