The Chiniak Bay Elder House, located on Providence Kodiak Island Medical Center’s campus, is designed to provide a warm, home-like environment for residents. It features 22 bedrooms, arranged in a duplex model with 11 bedrooms on each side. Our residents receive exceptional medical care, a variety of therapy services and an engaging, supportive environment that encourages social connections. Our referral process helps us understand the applicant's medical history and current health status, ensuring we can provide the appropriate care and support.

Yes, to apply for admission to Chiniak Bay Elder House, a referral from the prospective resident's primary physician is required.

The primary physician or facility should send the following directly to our facility:

  • Approved LTC1 and PASRR from the State of Alaska
  • Current medication list
  • Face sheet
  • History and physical notes
  • Most recent POLST
  • Recent progress notes
  • Recent lab and diagnostic records
  • Recent therapy services notes

Referrals can be mailed directly to our location, hand delivered to Elder House or faxed to the admissions office at the number above.
