Step 2 - Complete Annually:

SEWA Student Intake Packet

*The Student Health Requirements & Additional Health & Safety Requirements (pages 10-11) of the Student Intake Packet must be filled out by the school representative, signed and dated attesting to the accuracy of the information provided.

Once the clinical rotation is approved, and the student intake packet is completed, the SEWA Student representative will submit for student account access (EPIC, Pyxis, etc.) You will receive an email once your student network account has been created.

Step 3 - Claim Network Account

NOTE: You cannot claim your student network account until the listed start date on your access letter. Your access letter will be sent by email. Please claim your account using account claiming instructions.

Step 4 - Complete Once:

EPIC Training & Attestation (only if you will receive access to the electronic medical record)

Pyxis Training & Attestation (Nursing students only or if you will need access to the medication dispensing system)

Returning students & faculty

Step 3 - Reactivate Network Account

If it has been more than 90 days since your last login, you will need to reactivate your network account: instructions.

Onboarding instructions: must be completed annually

The onboarding process and requirements are the same for all students and instructors, even those who are also current Kadlec or Providence employees. Academic Services does not access or request records from HR or Employee Health.

All student rotations and hands-on experiences, both clinical and non-clinical must be arranged through Academic Services and the Education Office(s).

Step 1

A clinical education agreement between the Academic Institution and Southeast Washington Providence ministries must be obtained and active prior to student onboarding. To validate the status of the facility contract, please contact the St. Mary Student Coordinator at PSMMCStudents@providence.org or Kadlec Student Coordinator at AcademicServices@kadlec.org.

Step 2

Initiate a student rotation request by completing the following form. This can be submitted by either the student or the academic representative. A SEWA Student representative will be in contact with you regarding your request via email.

For a group request, please fill out the SEWA Group Request and attach it to the form above. Only one Microsoft Form submission is required per group.

Step 3

Student Health Requirements & Additional Health & Safety Requirements (pages 10-11) of the Student Intake Packet must be filled out by the school representative, signed and dated attesting to the accuracy of the information provided.
