Leaving the hospital doesn’t mean having to leave your care behind. We’re here to help with the transition after you’ve been discharged or if you’re between doctors.

If you enter from Goethals Drive, we’re across from the parking garage.
Yes, a referral is necessary. It helps us tailor and personalize any care you might need. We work closely with your primary care physician and specialists to make sure that you have optimal support for the continuity of your care.
We receive referrals from:
- Urgent care – if you have an urgent or ongoing medical need but aren’t established with a primary care doctor yet, you may be referred to our services
- Clinical care coordinators – if you’re not established with a primary care doctor yet, a clinical care coordinator may refer you to our services
Please bring the following to your appointments:
- Photo ID and insurance card
- Co-payment if required by your insurance plan
- Full list of medications with dosages, including over-the-counter supplements
- Detailed history of symptoms and treatments
- List of questions for the care team
