Whether you’re between primary care physicians or transitioning from hospital care to home, our dedicated team is here to ensure continuity and quality in your health care experience.

In the crucial first 30 days following hospital discharge, we’re committed to enhancing patient experiences and allowing a seamless healing process and stabilization of health conditions.

Our compassionate and skilled experts are dedicated to bridging the gap in your health care journey. Your expert, multidisciplinary care team may include:

  • Case managers
  • Clinical pharmacists
  • Doctors

Our unique approach is exclusively available through referrals to ensure personalized, coordinated care that is tailored to your needs. We prioritize your well-being and are here to guide you every step of the way.

Services We Offer

Leaving the hospital doesn’t mean having to leave your care behind. We’re here to help with the transition after you’ve been discharged or if you’re between doctors.

We facilitate appointments and coordinate comprehensive post-hospitalization care to ensure all aspects of your health are addressed.

Bridge Care can help you in several ways, including:

  • Address barriers to care, such as transportation
  • Connect you with available services in the area
  • Refer you to specialists, such as a dietitian or physical therapist
  • Schedule appointments for follow-ups or establishing care with a new primary doctor

By working closely with your existing health care physicians, we aim to reduce the likelihood of hospital readmissions and emergency room visits.

Additionally, we offer the option to act as your primary care physician for an interim period until you’re established with a new primary care doctor. This ensures continuity of care and a personalized approach to managing your health needs. Our integrated services strive to enhance your overall well-being by providing comprehensive support and coordination.

We provide short-term prescription refill request coverage to help manage your medication needs effectively until you’re scheduled to see a new primary care doctor.

Language should never be a barrier to quality care; interpreter services are available to support diverse patient needs.

Enjoy easy access to our facilities with complimentary parking, making your visits hassle-free.
