We’re more than just a medical fitness facility for patient use. Covenant LifeStyle Centre provides a wide range of services to its non-patient members as well, such as nutritional guidance, physical therapy, cardiac rehabilitation and pulmonary rehabilitation. Since 1984, we’ve proudly provided a friendly and non-intimidating workout environment that is open to the public.
Perched high atop the Covenant Medical Center east parking garage, the Covenant LifeStyle Centre is a full-service fitness facility with all the features to be expected in a modern gym, without any long-term contracts.

Registered dietitians are your most credible source of nutrition information. We employ a dietitian trained in the use of nutrition for preventing and controlling disease; they are available Monday through Friday from 8 a.m. to 5 p.m.

Our physical therapists at Covenant LifeStyle Centre are here to help build treatment programs that reduce joint pain and improve mobility and strength. We use a combination of techniques, manual manipulation and therapeutic exercises to target the source of your concerns.

We’re proud of the nationally certified cardiac rehabilitation program at Covenant LifeStyle Centre, which affords cardiac patients the opportunity to participate in an outpatient program that includes medical supervision, evaluation and instruction in physical activity education.
This personal treatment approach provides support to patients trying to make healthy lifestyle changes and can be highly beneficial for patients who have experienced any of the following diagnoses:
- Angina symptoms
- Coronary artery angioplasty or stents

- Heart failure

- Open heart surgery such as coronary bypass or valve surgery

Frequently Asked Questions
All of our therapy and rehab programs are located at Covenant LifeStyle Centre, on the sixth floor of the east parking garage of Covenant Medical Center, on 21st Street.
Yes, you do need a referral before scheduling an appointment with us.
If your referral is internal from another Covenant Health physician, you don’t need to provide us with additional medical information.
If your referral is from a physician outside of the Covenant Health network, you’ll need to bring all relevant medical records and imaging results. We can’t access these records in your Covenant electronic medical record (EMR) if they’re from an external referral.
Please arrive 15 minutes before your scheduled appointment to fill out paperwork. Your initial visit will last between 45-60 minutes. We’ll discuss your medical history, any specific concerns or impairments and the functional goals for therapy.
Dress in comfortable clothing and be prepared to participate in basic movement assessments and exercises during the initial visit evaluation.
Please bring the following to each appointment:
- Your photo ID and insurance card
- Co-payment, if required by your insurance plan
- For physical therapy or rehab: comfortable clothing you can easily move in
We’re excited for you to join our community at Covenant LifeStyle Centre!
Our process is as follows:
- Complete the enrollment forms. Depending on your answers, you may need a Medical Clearance letter from your primary care physician.
- Once the enrollment forms have been received by us and the $20 enrollment fee has been paid, we’ll schedule an entry evaluation with you.
- Membership dues will be pro-rated from your enrollment/start date until the next billing cycle.
When you first enroll with us, we will go over all the rules of the Covenant LifeStyle Centre and what we expect from each member. Some policies we ask members to remember each time they visit are:
- Each member will be screened as they enter the facility.
- Members must wipe each piece of gym equipment immediately following use.
- Parents or guardians must always accompany children 12-16.
- The swimming pool and outdoor track close 30 minutes prior to closing time.
Current members are allowed to bring one guest. If the guest has a free one-day pass, they don’t need a member to accompany them.
You can pay your monthly dues through our online bill pay system.
Yes, we can deduct the monthly dues from your Covenant paycheck. You’ll need to fill out a Payroll Deduction form, which you can request from our front desk.
Please note that this deduction will continue to be made until you discontinue your membership and have filled out the Membership Resignation form.
