Frequently Asked Questions
Covenant Outpatient Therapy is on the third floor of the Covenant High Plains Surgery Center on 22nd Street.
Yes, you do need a referral before scheduling an appointment with us.
If your referral is internal from another Covenant Health physician, you don’t need to provide us with additional medical information.
If your referral is from a physician outside of the Covenant Health network, you’ll need to bring all relevant medical records and imaging results. We can’t access these records in your Covenant electronic medical record (EMR) if they’re from an external referral.
Please arrive 15 minutes before your scheduled appointment to fill out the necessary paperwork. Your initial visit will last between 45-60 minutes. We’ll discuss your medical history, any specific concerns or impairments and the functional goals for therapy.
Dress in comfortable clothing and be prepared to participate in basic movement assessments and exercises during the initial visit evaluation.
Please bring the following to each appointment:
- Your photo ID and insurance card
- Co-payment, if required by your insurance plan
- Comfortable clothing you can easily move in
