At the Outpatient Rehabilitation clinic located within Providence St. Joseph Medical Center, our specialists work with your doctor to find therapies that help improve activities of daily living.

Outpatient Rehabilitation is on the first floor of Providence St. Joseph Medical Center, located at 6 13th Ave East.
Yes, you do need a referral before scheduling an appointment with us.
You can ask your doctor to send us an order for occupational therapy, physical therapy and/or speech therapy evaluation and treatment.
We’ll contact you to schedule your first appointment.
Please arrive 15 minutes before your scheduled appointment to fill out the necessary paperwork.
Your initial visit will last between 45-60 minutes. We’ll discuss your medical history, any specific concerns or impairments and the functional goals for therapy.
Please bring the following to each appointment:
- Your photo ID and insurance card
- Co-payment, if required by your insurance plan
- For occupational and physical therapy appointments: Comfortable clothing you can easily move in
We accept many health insurance plans and bill most insurance plans directly. Since coverage can vary, it’s best to confirm with your insurance provider directly before you schedule an appointment, so you aren’t surprised by unexpected charges.
Please bring your insurance card to all appointments.
Our office doesn’t offer sports medicine services; however, Providence St. Joseph Medical Center does have a Sports Medicine department in the same building. They offer many services and programs related to concussion management, sports injuries and return-to-play assessments.
