• Step 1: Gather vaccination and titer records
  Requirements are listed in the Student and Faculty Clinical Passport to be filled out and verified by your school once you obtain copies of your vaccination and titer records.

  • Signed Non-Employee Confidentiality Form
  • Signed Code of Conduct Acknowledgement Form
  • Signed Acceptable Use Agreement Form
  • Substance-Free Attestation and Acknowledgement of Substance-Free Workplace Policy
  • Student, Resident, Fellow or Intern Registration Form
• Step 4: Complete the HealthStream e-Learnings on the Epic Learning Resources page
  • Use the Providence ID you received, upon claiming your network account to complete the Epic training at the link below.
  • Please make your way to the Epic Learning Resource (ELR) page HERE >Scroll down to STUDENTS and select the most appropriate training for your role.
  • When you complete this training please complete the PRMCE Epic Student Attestation.
