At PRMCE it is our policy that all students, residents, fellows, interns, and medical observers are required to be fully vaccinated and provide proof of COVID-19 vaccination prior to being onsite for clinical rotations.

After you have reviewed the below documents, please print the following forms, review and then sign.

• Signed Non-Employee Confidentiality Form
• Signed Code of Conduct Acknowledgement Form
• Signed Acceptable Use Agreement Form
• Substance-Free Attestation and Acknowledgement of Substance-Free Workplace Policy

Please submit all completed forms and a professional JPG headshot to Joseph Spellman prior to start date.
