You can make your appointment with a Providence Medical Group clinic online or by calling 855-477-6864 Have your insurance information handy, and be ready to answer questions about your health history and prescriptions. 

Insurance and billing information

Providence Medical Group clinics accept most health plans, but please check with your insurance provider to find out what your plan covers. Please bring your insurance card to all appointments. 

We bill most insurance plans directly, but you’ll need to cover your co-payment when you come for your appointment. If you don’t have insurance, we ask for a deposit at the time of your visit. We’ll bill you for the balance. We accept personal check, cash, Visa or MasterCard. If you need help with your bill, call our Physician Billing Office at 866-747-2455.

What to bring to your appointment

Please come 15 minutes before your scheduled appointment so that you have time to check in and fill out any necessary paperwork. You should bring:

  • Photo ID and insurance card
  • Co-payment (if your insurance requires it)
  • Referral from your primary care doctor (if your insurance requires it)
  • List of medications with dosages, including over-the-counter medications and supplements
  • List of questions for the doctor — this ensures you get your questions answered

During your appointment, your doctor will ask you about your health history, symptoms and the treatments you’ve had in the past. Feel free to bring any information you think is helpful for your doctor to know. And please bring a list of questions to your appointment. It’s important to us that we answer all of your questions — and we know how easy it can be to forget them once you’re here.

Comprehensive Maternity Services Close to Home

At Providence Regional Medical Center, our Pavilion for Women and Children offers comprehensive maternity services and care options for families having babies, including obstetricians, certified nurse midwives, the Centering Pregnancy Program through the midwives, family practice providers, birth and family education, specialists in maternal-fetal medicine, and a Neonatal Intensive Care Unit (NICU) staffed by Seattle Children’s providers.
